With the default configuration set up in this how too I also believe that you need to put in a NAT table rule for masquerade like so:

iptables -t nat -I POSTROUTING -o eth0 -j MASQUERADE

Go through the "Setting up Alpine" down to "Certificates"

Then

   wget https://raw.githubusercontent.com/captainwasabi/openvpn-install/master/openvpn-install.sh
   apk add bash
   bash openvpn-install.sh

Follow the prompts.
